Be careful of any forms (Advance Beneficiary Notice of Noncoverage) you may need to fill out regarding payment with these folks. Check option 2. They already know your medical will not pay for the anesthesia, but they don't tell you that. You're handed the form 5 minutes before procedure and you are expected to make a split second decision. Call your provider ahead of time, if you have that luxury, and find out if Anesthesia is covered for your procedure.
