I'd appreciate the P Ward BMC office that they have transferred / shifted the health department to Amizara Building. When I visited to this office, I found that Marriage Certificate registration, Birth Certificate registration is done in this centre. The one who is interested to get a birth certificate copy for their kids who comes under P Ward, they can collect the Birth Certificate Form from the actual BMC P Ward office i.e. located opp. Liberty Garden & there is no charge for the form to register. A person is entitled to get end number of Birth Certificate copies according to the need & as of date 1 copy will cost for 6/- Please note, that fill the form properly and attached the valid proofs along with it like Discharge Card, Born Certificate issued by Hospital, Parents Aadhar Card (any 1), also, take a xerox of the application form, as the officials will ask you during the submission process. Once all the necessary documents are ready and attached, submit it to the official who is available in the office and they will let us know the day & time to collect the Birth certificate copies. Hope the above information will help to the one who is in need. -------------------------------------------------------------------- So as per the appointment given by the health department, I visited to their office between 9:00 am to 4:00 pm on 14/05/2018, one of the department member gave me my application form xerox along with a reference number mentioned on it and I was instructed by them to take the xerox to the main BMC office to make the payment and to collect the certificate. I visited the main BMC office, the staff members were very polite and helped me to get the birth certificate very easily. Overall experience, I must say BMC have taken some necessary steps and made it very easy for us to get the birth / death certificates.
