Primary Care Gold Coast, trading as Gold Coast Primary Health Network, (GCPHN) has been funded by the Australian Government Department of Health as part of national health reforms, and has replaced Gold Coast Medicare Local with the key objectives of: - Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes within the Gold Coast region; and - Improving coordination of care to ensure patients receive the right care in the right place at the right time Gold Coast Primary Health Network (GCPHN) plans to achieve these objectives by working directly with general practitioners, the cornerstone of primary healthcare and other primary health care providers, secondary care providers and hospitals to ensure improved outcomes for patients. In addition, PHNs will undertake extensive population health planning to identify service gaps and inform local strategies to meet these needs. Initial activities include: - Preventable hospitalisations - Childhood immunisation - Cancer screening - Mental health treatment rates GCPHN will also be implementing the Federal Government’s six key priorities for targeted work. These are mental health, Aboriginal and Torres Strait Islander health, population health, health workforce, eHealth and aged care.
