It is behind Rajesh Tilak hospital, Ample space to park 4 wheeler as well outside office. I went there to register birth certificate for my new born. Note down the instructions they gave me. Things you need - Come here 1 week after your child birth, You need to know the hospital name your child born and it's address, Birth date, Your name and wife name as per the Aadhar ( to avoid confusion later while applying for Passport), Your local address, Mobile number, Baby name, Your signature, RCHID or the card number you received from Suhadhaara maiyam already when the lady is conceived (it is important), a pen to write all these in the form they give you. Please note that you need to write some information in tamil and english. So, practise your name, baby names in tamil in home before coming. THey charged me 200 for one birth certificate. You can opt for more than one as per your choice. They give you a receipt with the date, when you need to come and collect the certificate. This particular department is there in a location inside office, go straight and last right room (opp to corporation officer room). They give you pad to fill the form. There is no charge for form though it says 1 INR on top. I am attaching the application form as well which you need to fill in the photos section here. Please check.
