My grandmother had a stay with you folks. She was extremely happy with the treatment she received. She wanted to make sure a few of your staff received recognition for jobs well done. Reggie your housekeeper kept her room spotless and she appreciated that. Also your wound care team kept her bandages dry and clean. Also your Activities Director Theresa kept her busy all the time, whether it was with word puzzles or great conversation. My grandmother always looked forward to Theresa’s visits. She also said the food was “delightful.” Please let your staff know that their hard work was appreciated by my grandmother.

I want to write this review to give your facility the recognition it duly deserves. Your staff made my mothers stay so easy for her. Theresa your Activity Director came into her room everyday to check on her. She always put a smile on her face, so much that she always looked forward to seeing her! I thank her from the bottom of my heart for making her days just a little easier! Also I would like to thank Reggie for keeping your facility so clean! You always hear horror stories about cleanliness of rehab centers, but not at your facility, it really is spotless!! To all your nurses, they all were great with my mom. They took time out to explain things with her and that mattered to her. I would recommend your facility to anyone that is looking for a rehab center.

My mother has been a resident at Warren Barr Lincoln Park for approximately 10 years. I have always been pleased with the attentiveness of the nursing staff and social workers. Most notably, Theresa Palmer( Activities Director) makes sure that my mom receives top notch services! She makes sure that she is registered to vote, provided with a beautician and participates in activities as tolerated. Overall, I appreciate the staff and their willingness to accommodate families during the stressful time of Covid-19.

Do NOT send any loved ones here. Starts at the top with Administrators who fake it and pretend to listen to your concerns, then do nothing. This includes head of nursing and Administration. The doctors are sub par. And changed my grandmother’s neurologist dosing without reason or warning. The nurses, other than one , are rude and uncaring. This place is dangerous; please stay away.... we were so worried about my grandmother while she was here.

This place is absolutely disgusting. From the quality of care, customer service, food, nurses, cnas, Jen the admin and the other staff members. During the two weeks my mom has been there she's had food poisoning twice and taken to the hospital. This place needs to be destroyed. The lack of care is heartbreaking. Do not send your love ones here, you will be disappointed. You can even try reaching out to Legacy for support, but you'll be wasting even more time.
